In 2372, Lenara came to Deep Space 9 to conduct field tests on creating artificial wormholes. There she met Jadzia Dax, whose past host Torias Dax was married to Lenara's past host Nilani Kahn. The two Trill females began Torias and Nilani's relationship again, risking being discovered and being exiled from Trill society for breaking the reassociation taboo, which would lead to the eventual death of the Dax and Kahn symbionts. As her brother's surname was Otner, it is most likely her birth surname as well, barring them not being full siblings or some other similar situation.
