'Chavez' told him that "when his heart started beating again, he would know". April managed to start the watch ticking again, but when Cortez fell from the MTI Building, it stopped. The significance of this is not clear, but obviously the implication is that he is now dead. It must be remembered, however, that when Westhouse gave it to her it was not yet ticking again. Cortez is a mysterious character who often talks in riddles. He has a mischievous sense of humour but takes his guidance of April very seriously. Cortez then tells April he has not seen Tobias since he was a young Istrum, and is pleased - although unsurprised - to hear that he has become Vestrum. Just before she accidentally Shifts to Arcadia (in her underwear), April has a dream where Cortez is captured by Gordon Halloway, and later has another dream where she sees him imprisoned in some sort of enormous machine. When she is finally reunited with Cortez, she discovers this actually happened: Cortez says Halloway captured him using a "small army". April's vision of Charlie in the Spirit Dig also mentions how "Cortez the Red" is watching out for her. His relationship with his brother, Jacob McAllen, the Green Kin, is a bitter one. Cortez scorns the life McAllen has chosen, even mocking his attempts to convince Cortez to join him by saying he talks like a badly written play. The Brute, wielding a Gravity Hammer, attempted to smash O'Brien, but the Spartan managed to push him out of the way and take the brunt of the blow herself. After he and his fellow marines knocked down and killed the Brute, O'Brien finished the mission himself. With her dying breaths, the Spartan, Cal-141, handed Cortez the data she had gathered from the strange ruins scattered about the planet and asked him to bring them to Dr. Halsey. It is unknown whether he fulfilled this request.

Er taucht auf seinem Geisterschiff Black Skull auf, welches sich in einer Höhle auf der Insel Isla Corsaria befindet, wo Mario, Bart-omb und deren Freunde gestrandet sind. Cortez war früher einmal Pirat, doch er wurde von seiner Crew umgebracht, da diese den Schatz von ihn wollten. Nun ist er nur noch ein Geist bzw. Skelett, verdammt dazu, auf Ewigkeit zu leben und seinen Schatz zu bewachen. Mario trifft auf ihn während des Spielverlaufs, als er die Piratenhöhle durchsuchen wollte. Dabei hört man des Öfteren die Stimme von Cortez, die besagt, dass Mario aus der Höhle wieder verschwinden soll. Auf dem Geisterschiff angekommen müssen Mario & Co. gegen ihn kämpfen, da er glaubt, dass sie seinen ganzen Schatz stehlen wollten. Cortez hat zwar nur 20 KP, doch der Schein trügt: Insgesamt muss man nämlich gegen 3 Versionen von Cortez antreten, welche natürlich von Mal zu Mal stärker werden. Kurz nach dem Sieg über ihn überreicht er Mario bedingungslos das Sternjuwel, da es für ihn keinerlei große Bedeutung hat und er es eher als nutzlos empfand. Cortez kämpft in mehreren Formen gegen Mario, in seiner ersten Form hat er vier Arme und einen Beinhaufen. Wurde letzterer besiegt, ist es leichter, Cortez am Kopf zu treffen. Am besten greift der Spieler mit Mario Cortez und mit dem Partner den Beinhaufen an. In seiner zweiten Form nimmt Cortez in jede seiner vier Hände eine Waffe mit 4 KP, welche den Getroffenen auch vergiften kann. Es handelt sich um ein Rapier, einen Säbel, einen Enterhaken und ein Schwert. Das Rapier hat eine Angriffskraft von 3, die anderen Waffen eine Angriffskraft von 2. Alle Waffen können mit Aeronas Hurrikan, Barbaras Feuerzauber oder einem Donnerzorn gleichzeitig besiegt werden. Weiter geht es nur noch gegen Cortez' Kopf, wenn seine Form mit den Waffen besiegt wurde. Diesmal trägt er eine Kette mit einem Medaillon, das an einen riesigen Diamanten erinnert, und beißt Mario oder seinen Partner. Nun kann er mit starken Angriffen besiegt werden, aber es sollten keine Sternen-Attacken benutzt werden, da man die Sternen-Energie erst nach einem weiteren Bosskampf gegen Crucillus wieder herstellen kann. Nachdem Mario und die verschollenen Personen wieder draußen im Freien sind, wird die Insel plötzlich von einem Kanonenschiff der Crucionen angegriffen unter der Leitung von Crucillus. Um den Angriff zu stoppen, versuchen Mario und Korsario, Cortez dazu zu überreden, seine Galeone für den Kampf nutzen zu können. Dieser lehnt das Angebot zuerst ab, doch als er das Schädeljuwel von Korsario bemerkt, versuchen die beiden, miteinander zu verhandeln. Auch wenn es am Anfang noch nach einem 2. Kampf mit Cortez aussieht, da das Juwel eigentlich in seinem Besitz war und er es wieder zurückhaben will. Am Ende bekommt er das Schädeljuwel, im Austausch dafür soll Cortez den Angriff der Crucionen abwehren. Es kommt zu einer Seeschlacht auf dem offenen Meer, aus welcher Mario und Cortez als Sieger herausgehen. Letztendlich fährt er Mario und die Anderen wieder zurück nach Rohlingen. Danach fährt Cortez Mario und seine Partner jederzeit von Rohlingen zur Isla Corsaria und wieder zurück. Um die Fahrkarte für den Glimmer-Liner zu bekommen, muss er Mario erneut zur Insel bringen, um Palmina und Romanzo abzuholen, weil Palminas Vater die beiden vermisst. Als sie wieder in Rohlingen sind und Mario Al Palmone besucht, bekommt er die Fahrkarte.
