Augmenté de deux musiciens, Ivan Krall (basse) et J.D. Daugherty (percussions), il publia son premier album, Horses en 1975. L'album commence par une reprise de Gloria, composé par Van Morrison, qui débute par ces mots: "Jesus died for somebody's sins ... but not mine". Le second album du groupe diffusé en 1976, Radio Ethiopia, fut perçu comme « self-indulgent », et la carrière de Patti Smith connut un coup dur lorsqu’elle se brisa la nuque en tombant sur scène, au début de l’année 1977. L’album suivant, Wave (1979), ne connut malheureusement pas le même succès. Entre-temps, Patti collabora à trois albums du Blue Öyster Cult avec Allen Lanier. La rockeuse se marie alors avec l’ancien guitariste de MC5, Fred "Sonic" Smith, et se retire de la scène musicale pendant la quasi-totalité des années 80 pour élever ses enfants. Elle refait son apparition en 1988 avec Dream Of Life, puis sa carrière connait une nouvelle interruption, due à une suite d’événements malheureux dont la mort de son mari. Sombre et mélancolique, son album suivant, Gone Again (1996), fut souvent décrit comme destiné à exorciser le spectre de cette période malheureuse.Cet album fut réalisé avec deux membres du Patti Smith Group d'origine : Lenny Kaye (guitare) et J.D. Daugherty (percussions), avec l'adjonction de Tony Shanahan (basse) et de Luis Resto (claviers). En 1997, le Patti Smith Group composé de Lenny Kaye, J.D. Daugherty, Oliver Ray et Tony Shanahan sort Peace And Noise, qui marque le retour au son de ses premiers albums; les mêmes se retrouvent sur Gung Ho (2000), suivi, en 2004, de Trampin' , album très marqué par la guerre en Irak. Patti y affirme son pacifisme et même sa voix prend des accents à la Joan Baez, elle affirme sa compassion pour le peuple irakien. En 2004, Patti Smith fait une tournée (Trampin' Tour) en France, elle passe notamment par le Festival des Vieilles Charrues ainsi qu'au Festival Art Rock à Saint Brieuc le 27 mai, et en Italie. Le 19 mars 2006, elle donne un concert public pour la clôture des jeux paralympiques à Turin. Elle refait une tournée française en 2007.
