Under the effects of the genjutsu, any pain, no matter how small can be relocated to any point of the victim's body. This technique was originally created to apply in combat between the members of the Uchiha clan, where they would use it to relocate the pain to the other's eyes and give themselves the advantage within the dōjutsu war. Much later, when the intern conflict had since long ceased, the technique ended up in the hands of Shisui Uchiha, who polished and perfected it into a weapon of true horror. Using his expertise, even the smallest of scratches could be grotesquely enlarged and distributed throughout the entire victim, to the point they would become paralyzed by pain. This excruciating pain would then easily allow Shisui to smash through any Mental Barricades and enhance his grip on them even further. This would then allow to start manipulating his victim's sense of time, which permits him to stretch out this enhanced pain to hours or even days, similar to his friend's infamous genjutsu. Because the genjutsu, no matter how inflated, still found root in actual pain, the illusion became incredibly hard to break, as it was not necessarily "untrue". As expected, it was useless to utilize pain to break the technique and the horror one experienced during the technique made building up enough concentration to break it a nigh impossible task as well. The end result was a genjutsu that was triggered by a scratch and a gaze but resulted in a monstrous experience, enough to permanently traumatize the victim. The fact that Shisui always managed to be creative in his executions and interrogations, to the point of even simulating crucifixion, made the illusion that much stronger as well.
