The Cairn were a humanoid telepathic species who were so far advanced telepathically that they had lost the ability to communicate verbally. Unlike most other telepathic species, Cairn telepathy sent images rather than words, allowing for a much higher rate of information exchange. Due to this fact, other telepathic species had difficulty communicating with the Cairn, though it was possible with training. Privacy was a concept unknown to the Cairn before they started to interact with the Federation.

Cairn appears to be a pile of three rocks, each one floating on top of one another. Each of the three rocks Cairn is composed of is a different color: the bottom rock is greenish-brown, the middle is maize, and the top is red. The three rocks are also different sizes; the bottom is the largest while the top is the smallest. The top rock also has a pair of black eyes, making this enemy somewhat resemble a stone snowman.

The planets population had the highest addiction rate in the Milky Way Galaxy, approaching hundred percent in areas. The police force that Garryn Bek belonged to was also corrupt and under the control of drug lords. The hierarchy of drug lords was led by Kanis-Biz, the father of Marij'n Bek and father-in-law of Garryn Bek.

Cairn was mainly inhabited by the aernar, a peaceful and religious race. However, minotaurs started taking over the territory, invading from the mainland. Most villages fell under the minotaurs' aggression. Now, the minotaurs mostly wage war among themselves, on the mainland coast or with other inhabitants of Cairn. One of their enemies are the nomadic hill giants that live in the interior of Cairn. In the center of Cairn stands Kasdim, an old Chaldean city, full of treasure and valuable artifacts. Today, Kasdim is home to monster factions which battle over control of these treasures.

Most food is imported because all agriculture on Cairn was devoted to the production and manufacturing of the drugs. When the Dominators arrived to conquer the planet, but the corrupt government surrendered some it's people, Cairn like many other worlds showed their compliance with the Dominators' plans When Garryn Bek escaped the Dominators along with Vril Dox II they devised a plan of ousting the drug lords from Cairn. The plan involved taking over the police force and using them to fight the private armies of the drug lords. There were good police officers followed Bek and Dox with question whilst the corrupt cops were scared into action by the bounty hunter Lobo into joining Dox. The founding L.E.G.I.O.N. were being treated by their host, Kanis-Biz, at his estate prior to the assault. When the assault hit, they were reluctant to join in on another one of Dox's deceits. They fought Lobo, while Dox held Kanis-Biz hostage. Vril Dox II forced his surrender after Kanis-Biz saw that the battle was hopeless because Lobo was fighting against him. After Kanis-Biz ordered all of his troops to stand down and surrender Vril Dox II executed him, ending the rule of Kanis-Biz as drug lord.

According to Star Trek: Star Charts ("United Federation of Planets I"), the Cairn were from the planet Cairn, and, by 2378, this planet had become a member of the Federation.
