Deathmaiden (no real name known) is a bioterrorist who appears to come out of hiding roughly every 20 years or so to devastate an area with a new plague, is seemingly stopped for good after a few years only to reappear when it is time again. Her appearances so far were in 1929, 1949, 1968 and 1989. Her attempt to wipe out New York in the early 1930s was stopped by Macrobiotic's grandmother. She killed Macrobiotic's father with a letterbomb, who had been working on stopping her attempt to wipe out central Africa with a plague.

The precise mechanism by which Deathmaiden keeps returning is as yet unrevealed. Three known thesis are cloning where the clones are aged normally to minimize side-effects, a non-vertebrate regeneration process and some form of self-replicating cancer cells which are held by Macrobiotic's mother, grandmother and great-grandmother respectively.
