On Madhouse difficulty, the Survival Knife is moved from its original position to being stuck in a table inside the Recreation Room on the first floor (Wrecked Ship 1F). In this location, a Corrosive is required to open the door to the Rec Room and retrieve the knife. After rescuing Ethan, the survival knife along with what were in Mia's inventory are automatically transferred to the Item Box for Ethan's use. Similar to Ethan's folding knife, it has weak damage output (though noticeably albeit slightly better than the pocket knife) but compensates with its slightly longer range. In Ethan Must Die mode, the survival knife is several times more powerful than the one found in the story, easily clearing hives and taking down normal molded (albeit still highly risky).
