- The Hollow Earth, sometimes called the Inner Umbra, is a Realm or set of Realms occupying the inside of the planet Earth. It is speculated to have some connection to the Shade Realm of Matter. Gravity and other physical laws work differently within Hollow Earth, and it is still under active exploration by both the Sons of Ether and the Void Engineers.
- Elizabeth Sherman leaves the B.P.R.D. to find peace only to be captured. Abe Sapien, Kate Corrigan, Roger and new recruit Johann Kraus travel to the arctic to rescue her.
- Liz har gett sig av upp i Uralbergen för att besöka ett uråldrigt kloster och där försöka finna inre frid. Samtidigt planerar Abe att göra som Hellboy och lämna B.P.F.F.. Kate försöker få honom på andra tankar men han verkar bestämd. När Abe håller på att packa uppenbarar sig en avbild av Liz för honom som bönfaller honom att hjälpa henne. Abe skjuter på beslutet att lämna byrån och tillsammans med Kate, Roger och byråns senaste tillskott Johann Kraus beger han sig i stället ut på en räddningsoperation.
- The city of Praxis is located there, and its guardians patrol the surrounding area. There are multiple gateways to Hollow Earth that can only be opened by a keystone. All the entrances to Hollow Earh are equipped with sensors and vampire detectors, and if they detect vampire blood, then a laser field activates which forces the intruders down the vast network of granite tunnels to the nearest Praxian Garrison. The tunnels within Hollow Earth are well lit due to many pillars that outline the cave systems.

There are many vicious abnormals within Hollow Earth, many of whom are extinct on the surface, including the Basilisk. Humans and abnormals not only live in the city but also in the outlands where they live in nomadic cultures or outland tribes and move to seasonal camps to farm large mushrooms called Rashi. All beings within Hollow Earth are ruled by Praxis, often with an iron fist, although it has been noted that Praxis's harsh laws are the only thing keeping Hollow Earth's dangerous abnormals from overrunning the entire planet. The rock surrounding the city contains a high percentage of lodestone and magnetite which helps with their communications but also acts as a natural electromagentic shield, but the supply tunnels a few miles above the city are primarily granite in composition. The only way to enter Praxis is by airship or by one of the outland gates.

När de kommer fram till klostret möts de av lämningarna av ett blodbad där samtliga munkar brutalt har dödats. Abe finner Liz livlös på golvet. Johann använder sig av sina elektoplasmaegenskaper för att komunicera med kroppen och konstaterar att den inte är död utan bara ett tomt skal. Det verkar som att hennes livskraft sugits ur kroppen på liknande sätt som hände under Giurescu historien (där den livlösa Roger suger i sig Liz kraft och vaknar till liv). På golvet i templet hittar det även en död varelse som efter Johanns undersökning visar sig vara från underjorden. Den finner en djup spricka i golvet och gissar att de där ska finna förklaringen till Liz tillstånd och kunna rädda henne. Abe, Roger och Johann klättrar ner i hålet och Roger bär den livlösa Liz i en bärkapsel på ryggen. Väl nere i underjorden stöter de på flera oväntade och dittills okända fenomen och varelser.
